Supporting communities in need—both locally and nationally—is a key way we fight for what’s right, and Affinity Groups are one way AHF fulfills this mission. In the United States, HIV disproportionately affects minorities, such as Black and LGBTQ+ populations. To combat this, AHF Affinity Groups provide specialized services and care where they are needed most. The launch of Impulse Group in 2009 marked the first initiative, paving the way for the five diverse groups we have today.

Impulse’s Executive President Jose Ramos (second to right) is joined by chapter leaders for the Impulse International Summit in 2014.
Impulse’s main goal is to break social barriers by promoting sexual and lifestyle wellness for gay men using modern approaches. Founded by Jose Ramos—now Impulse’s Executive President—in 2009, the idea emerged from a discussion with friends about the rising rates of HIV among gay men. AHF President Michael Weinstein encouraged Jose to “start a social revolution” with his concept, leading to the successful launch of the affinity group.
Impulse is now one of the world’s largest HIV/Wellness groups for gay men, reaching over 523,000 people in 2024. It operates in over 26 cities across 13 countries spanning five continents. The federation of chapters, led entirely by over three hundred volunteers, hosted over four hundred events in 2024, linked 1,050 people to care, distributed 80,800 condoms, and registered 1,100 new voters—all with the goal of reducing the stigma around STIs in gay men’s communities.

Each chapter tailors its activities and support to the unique cultural and HIV-related needs of its city while collaborating with other chapters globally to adapt to our rapidly changing world. For example, the Impulse Mexico chapter runs the Free2Be initiative in their Guadalajara office, providing a free 24/7 mental health hotline staffed by professionals. Callers in need are connected with free, professional in-person care.

Volunteers play a major role in Impulse’s success by building and maintaining relationships within the group and the broader local community. Social events centered around emotional well-being—such as themed parties, community roundtables, drag brunches, and more—are paired with health initiatives like the Gay Men’s Sexual Trauma Treatment Network, educational newsletters and online games, and high-level advocacy fighting for LGBTQ+ rights.
